Since we are a non-profit organization, we rely
heavily on the community's support for donations
and subscriptions.
Our subscription drive helps us financially with
supplies, materials and equipment we need, so
that we can give the community the service it
deserves.
Being a subscription holder helps us as well as it
helps you. What this means to you, if you need
emergency services we will bill your insurance
company and the remaining balance that your
insurance may not cover is waived. That is our
thanks for you supporting us. This subscription is
for emergencies only and is not intended for wheel
chair transport services, but we can offer a member
rate for wheel chair van services.
Subscriptions are sent out annually to all the
residents of Forks Township, followed by reminder
notifications. If you have not received one or have
just recently become a member of our community
and would like to become a subscription holder,
you may do so by calling us at (610) 253-7416.
For your convenience, just click on the image to the
right and you will be able to print out a full page
subscription form. Just fill it out and drop it in the
mail. Or if you would like you are more than
welcome to come to pick one up and meet the
members of Forks Emergency Squad.
